One-Page Profiles: A Simple Tool for School, Medical, and More

This joint training with Oregon Family to Family Health Information Center provides a well-rounded look at how a one-page profile can be used across multiple settings.

More than a Chart: One-Page Profiles for the Medical Setting

Learn how a powerful, yet simple, tool can introduce health needs in a positive, respectful way. Help staff of medical, dental, and therapy offices, as well as social workers, coaches, and others understand a person’s unique personality and needs.

One-Page Profiles for School and Beyond

Learn how a powerful, yet simple, tool can introduce strengths, what works and does not work, and a vision for the future in a positive way. We’ll explore trajectory and person-centered planning, walk through how to create a one-page profile, and identify the many places you can use one to help others understand a person’s unique personality and needs.

Simultaneous trainings will be provided in English and Spanish. Please request other language supports at least 14 days before the event by emailing contact@oregonfamilytofamily.org.
