
One Page Profiles: A Person-Centered Planning Tool
Seeing your child’s strengths and having a strong vision for their future are important parts of keeping your child at the center of decisions and on a positive trajectory to achieve that future.
One Page Profiles are great tools to capture what you know about your child’s strengths, gifts, and talents and to share their hopes and dreams, as well as what works for them.
You and your child can share their One Page Profile at school, in decision-making meetings, with health care providers, as well as with community, extra-curricular, or peer groups important to you and them.
What can I include in a One Page Profile?
Child’s Name and Age
Grade Level
Child’s favorite things or activities
Child’s strengths and special attributes
What works for your child, and what doesn’t work
What’s important TO me, and what’s important FOR me.
Goals and family/youth vision for the future
What else should I consider?
Make different versions as needed… one for school, one for doctor’s visits, one for recreational activities, or one for your youth to share when looking for a volunteer or paid job… the possibilities are endless!
As your child grows, get them involved!
Edit at least once a year to keep it fresh and relevant to your child.
Keep it simple. Make it fun.
